If you paid an additional deposit due to your adopted pet not being fixed (only adoptions prior to July 31, 2022), please fill out the following form for reimbursement after the surgery is completed. Proof of surgery is required and the certificate will have to be uploaded below. Refunds are processed within 7 business days.
